A. Application for New Service 1. Each applicant for new service will be required to provide the company with the following information: a. name of applicant and spouse or business occupying premises to be served; b. whether applicant is an individual, partnership, association, corporation, or government agency; c. date of application; d. location of premises to be served; e. date applicant will be ready for service; f. whether the premises have been heretofore supplied; g. purposes for which service is to be used; h. address to which bill are to be mailed or delivered; i. capacity of applicant-owner, tenant, agent, or other (specify) for premises to be served; j. rate schedule applied; k. phone numbers including home, mobile, work and fax if applicable; l. such information as the company may reasonably require as approved by the company. 2. Purpose: The request for service does not bind the applicant to take service for a period of time longer than that upon which the rates and minimum charge of the applicable rate schedule is based: neither does it bind the company to serve except under reasonable conditions. 3. Individual Liability for Joint Service Two or more parties who join on one application for service or Membership Certificate shall be jointly and severally liable for proper use of the company's service and for payment of bills and shall be billed by means of single periodic bills. A member making any material change in the size, character, or extent of the equipment or operations for which the company's service is utilized shall immediately file with the company a new application covering such change of service. 5. Connection of Service The company will endeavor to connect electric service as soon as possible, at the company’s discretion, after an application for service has been received; however, it will not be required to connect a service on the day the application is received if the connection cannot be scheduled during regular working hours for that day. Should a connection be required during a period other than regular working hours, defined by the company as Monday through Friday from 8:00 a.m. to 3:00 p.m. (excluding weekends, holidays and any such day observed by the company as such) at a member's request, the member will be required to pay actual costs associated with the connection, which have been established at a minimum charge for a connection under normal conditions, as defined in the Fee Schedule. B. Membership Certification 1. All applicants taking service from Wells Rural Electric Company shall become a member of the company. Membership will be effective upon providing the company with the pertinent information as set forth: a. name of applicant, spouse b. One form of identification. Identification shall be one of the following: social security number, driver’s license number, tax identification number, any other number unique to the applicant. 2.
